Who is requesting the records?

Records can contain protected health information. Requests are reviewed for identity, legal authority, and applicable privacy requirements.

I'm the patient

Complete the LREMS Authorization to Release Medical Records ↗, and provide valid photo identification as instructed by our office.

Contact us to confirm the secure submission method, required details, and any applicable fees.

I'm requesting records for someone else

Legally authorized representatives may request records with proof of identity and authority. Requirements depend on the relationship and applicable law.

  • Spouse or next of kin: Photo ID and appropriate legal documentation or authorization.
  • Healthcare power of attorney: Photo ID and relevant power-of-attorney paperwork.
  • Parent of a minor: Photo ID and birth certificate, guardianship papers, or other accepted documents.
  • Guardian: Photo ID and applicable court-appointed guardianship papers.
  • Executor, administrator, or attorney-in-fact: Photo ID and supporting appointment or authorization documents.

Being a family member alone does not necessarily establish legal authority. Call the office to confirm what documentation is needed for your circumstances.

I'm an attorney

Records requests generally require documentation authorizing the specific disclosure to the attorney, such as:

  • A completed, signed authorization identifying the requesting attorney, including the LREMS Authorization Form ↗.
  • A qualifying judge-signed subpoena authorizing release, when applicable.
  • A court order authorizing release.
  • A signed authorization from a legally authorized representative with proof of that authority.

All requests are reviewed under applicable privacy and legal requirements; submission does not guarantee release.

I'm with a government or enforcement agency

Law enforcement and other state or federal agencies should contact our office to confirm the designated process. Depending on the authority and purpose, an agency request, qualifying judge-signed subpoena, or court order may be required.

Any release is subject to applicable privacy laws and review.

I'm a healthcare professional or facility

Physician Certification Statement (PCS): This form is used by authorized healthcare personnel to document medical necessity for certain ambulance transports. Patients generally do not need to fill out a PCS.

LREMS / SCVEMS Physician Certification Statement

For other facility medical-records requests, contact our office for instructions specific to the disclosure.

I need records from St. Croix Valley EMS (SCVEMS)

SCVEMS records may require a separate Medical Records Release form. That form is not currently available for download here. Please call our office to obtain the correct authorization and submission instructions.

The PCS form under Healthcare Professionals covers both LREMS and SCVEMS; it is not a substitute for a records authorization.
